signing up with NAPKRA

Maybe somone can help me out here.
I filled out the form to become a NAPKRA menber and printed out the waiver. I remember reading that I need to send the waiver and a picture to ??? and the money to ??? well anyway I closed that window where the addresses were and now I cannot get back to it without regestering again (or at least I cannot find a way).
It would really be great if somone could send me the adresses or tell me where I can find them.
